Required Documents for Taking the Written Permit Test to be covered with students
- Must be 15 ½ years of age or older
- Must show your contract from Cowboy Driving Academy after completing class
- Have 1 primary ID (see pg. 1-1 of Oklahoma Driver’s Manual)
- Have 1 secondary ID (see pg. 1-2 of Oklahoma Driver’s Manual)
- Have Social Security card or know the number
- Have official "8th Grade Reading Prof., Enrollment & Attendance" letter from school
- Not required for applicants 18 & older
- taken in summer months, official school letter must have the upcoming school term year or parent must sign affidavit
- Parent or legal guardian must accompany applicant to sign authorization certification forms required by law.
- If not born in the U.S., must show valid documentation of legal resident status or proof of citizenship.
To take the driving test with Cowboy Driving Academy,
Student must meet the following requirements:
- Have permit with issue date at least 6 months prior to test date (if under 18)
- Be 16 years of age or older
- Have all fees paid in full
- Have certificate of completion of driver education course (if under 18)
- Have 2 forms of ID. 1st ID can be a permit; 2nd ID can be current school year ID with picture, social security card or know the number (this is how they will look your record up in the computer)...(see pg. 1-2 of ODM for list)
- Have a notarized Parent Affidavit (allows student to test without parent present)
- If not born in the U.S., must show valid documentation of legal resident status or proof of citizenship.
